Who Could Benefit From Diabetes Management?

Is diabetes related to your disability?

As per the guidelines set by the NDIS, funding for diabetes supports is available only if they are directly linked to your disability.

If your disability hinders your ability to independently self manage your diabetes, our nurses may be able to assist by providing a specific plan alongside training.

You don't have a diabetes management plan

If you don't have a plan and want reassurance for yourself, your family and your support staff, creating one with our Nurses will be highly beneficial for your diabetes management.

We can incorporate diabetes management either within your Care Support Plan or create a seperate document depending on your individual circumstances.

We include infomation such as:

• Medication administration
• Understanding blood glucose monitoring devices and interpreting the data
• How to manage hypo/hyperglycaemia specific to the individual
• Ulcer and other wounds related to diabetes and your disability

You have a plan, but it needs updating

As life changes, you may encounter changes within your health and disability. If you have an outdated plan, we can also assist with updating your current one.

We will complete a comprehensive assessment to ensure all changes are accounted for and any updated training is provided to you, your family and your support workers.
